DOS Finals Rescheduling Request Form
Students may use this form to submit a request to reschedule a final exam due to bunched finals or because of extenuating circumstances. All requests will be reviewed and students will be notified of the final decision by email.
Once a request is approved, the student and professor will be notified via email of the approval. The student will be encouraged to reach out to the faculty member to determine when the make-up exam will be scheduled. Per the Senate By-laws, “The student must complete all outstanding work on a schedule determined by the instructor, and by the end of the third week of the following semester.”
